We welcome referrals by medical or mental health providers, caregivers, case
managers, law enforcement professionals, educators, community agencies, clergy,
employers, hospitals, family members, friends, former patients and patients
themselves. If you would like further information, call our admissions office at

What should I bring with me to the hospital ?
We recommend that you bring the following items:
» Comfortable, casual attire and shoes.
» Identification, insurance cards and other medical history if available.
» Overnight personal care items including shampoo, toothbrush and toothpaste.
» Personal items.
What things should I leave at home ?
Please leave the following items at home:
- Any metal objects, including disposable razors, car keys, mirrors, glass bottles, pocket knives, wire coat hangers, anything with straps or belts or glass.
- Television, radio, cd or cassette players, cellular phones, pagers, curling irons, blow dryers, electric razors with cords, alarm clocks.

assessment ?

Yes, Glen Oaks Hospital encourages family involvement.
How do I pay for my treatment ?
Glen Oaks Hospital accepts Medicare, most commercial insurance, private pay, and
Medicaid for individuals through 20 years of age. If you are unsure about your
coverage, you may call us to determine your eligibility.
Does Glen Oaks offer an out-patient program ?
Glen Oaks Hospital offers an intensive, traditional out-patient program
customized specifically for chemical dependency and dual diagnosis. The sessions
are held Monday, Tuesday and Wednesday from 6:00pm - 9:00pm. To set up a time
